I/we accept ___________________________________ (“the cat”) a __________________(F/M) cat who is approximately_______________________months/years old, and Paws4Survival Rescue Inc. accepts our offer to adopt the cat.

In consideration for adopting the cat, I/we hereby agree to the following adoption conditions:

1. To consider and treat the cat as a household companion, not as an outside pet, and to provide him/her with humane care and treatment, including proper food and clean/fresh water, exercise, grooming, and a reasonable amount of time to adjust to his/her new home and surroundings.
2. To keep the cat indoors at all times. 
3. To never get the cat declawed or alter their appearance in any way, shape or form.
4. To provide routine veterinary care in the form of annual vaccinations, monthly prevention treatments as recommended by a veterinarian, and emergency medical care when needed.
5. To NEVER sell or give away the cat to another person, relative, or any other individual or rescue group, humane association, shelter or pound, or any medical or experimental laboratory or any other organization. Instead, return the cat to Paws4Survival.
6. To immediately notify Paws4Survival in the event of the cat's death within the first year of adoption. 
7. To immediately notify Paws4Survival of any change of address or telephone number, or of any plans to move from my/our current residence during the first year from the signing of this agreement, and notify Paws4Survival of any change in circumstances which may prevent me/us from keeping the cat or comply with any provision of this agreement.
8. I/we understand that the cat has been checked, and vaccinated by a veterinarian and that his/her state of health is accurately represented to the best knowledge of Paws4Survival. However, if I/we wish to, we can have the cat examined by my/our own veterinarian within ten days of adoption.  If upon such examination it is determined that the health of the cat is not satisfactory, I/we may return the cat within a week to Paws4Survival. In any event, I/we understand and agree that if I/we choose to return the cat, the adoption fee is not refundable.
9. I/we understand that all animals can carry and transmit diseases, some of which affect people, such as bacteria, viruses, parasites, and ringworm, and that these diseases may be undetectable at the time of adoption. I understand that ringworm has a 7 to 21 day incubation time and that a cat can be a carrier and break out with it. I understand ringworm is contagious to me and other pets in the household.
10. I/we understand that my newly adopted cat may develop a cold, also known as the herpes virus, after bringing my cat home. All cats have the herpes virus; however, many do not exhibit symptoms until they are stressed. Going to a new home can be stressful for some cats. I understand that if my newly adopted cat develops a cold, it can potentially give the cold to any of my current cats. Symptoms are similar to a human common cold such as sneezing or runny eyes. If the cat develops more severe symptoms such as thick discharge, a fever, or becomes lethargic or is not eating, it must be seen by a vet.
11. I/we accept the animal as it is at the time of adoption and understand that Paws4Survival is not responsible for any medical conditions not readily detected prior to or at the time of this adoption or discovered after such adoption.
12. I am aware that pets may exhibit normal but potentially undesirable behaviors including, but not limited to, aggression, house soiling, biting, scratching (people, furniture, and woodwork), digging, mounting, spraying urine (cats), and that these normal behavior patterns may be difficult to manage. No one at Paws4Survival has told me that this pet will not engage in any of these behavior patterns.

 

I/we understand that no representations are made by Paws4Survival as to the temperamental or mental disposition of the cat. Accordingly, I/we hereby adopt the cat at my/our own risk, and indemnify and release Paws4Survival, its agents, volunteers, Officers and Directors, of any and all liability arising from damages to person(s) or property caused by the cat.

I/we have read and fully understand the terms and conditions of this agreement, and agree that it is not arbitrary and is common in pet adoption contracts.  I/we understand and agree that if I/we fail to comply with any of the terms specified herein, Paws4Survival HAS the right to reclaim the cat without notice or personal service.  I/we further agree to pay $2,000.00 in liquidated damages to compensate Paws4Survival for the cost of enforcing this contract in a court of law.

By signing this document I am agreeing to comply with stipulations contained in the Adoption Contract. I certify that the information I have provided in this application is truthful and complete.

I/we accept                                             (“the dog”) a                                                      (color) male/female dog whose breed is known as                                              and is approximately                 months/year(s) old, and Paws4Survival Rescue Inc., accepts our offer to adopt the dog.

 

In consideration for adopting the dog, I/we hereby agree to the following adoption conditions:

 

1)  To consider and treat the dog as a household companion, not as an outside pet, and to provide him/her with humane care and treatment, including proper food and clean/fresh water twice a day,  four daily walks , shelter, regular exercise, and a reasonable amount of time to adjust to his new home and new surroundings.

2)  To provide routine veterinary care such as yearly checkups, prevention treatment and emergency medical care for unusual symptoms such as loss of appetite, coughing, excessive urination, vomiting, diarrhea, blood in stool or urine, inability to walk, arthritis, allergies, skin conditions and lethargy; and to provide regular grooming and socialization.  

3)  To register the dog with and obtain a dog license from the municipality where I live or as required by law, and to   consult a veterinarian to tag the dog with a microchip.

4)  To safeguard the dog from loss or mishap. This means walking the dog on leash at all times, not allowing the dog to run at large unsupervised, not allowing the dog in the yard without human supervision, and keeping the dog as an indoor household companion at the premises listed as my address.

5)  If the dog is lost or stolen, to immediately notify Paws4Survival by phone and email.  If I/we are unable to reach the undersigned representative, to immediately call:  (978) 882-5847 AND to email Nicole.C.DiPaolo@Verizon.Com   

6)  NOT to give or sell the dog to another person, relative, or any other individual or rescue group, humane association, shelter or pound, or any medical or experimental laboratory or any other organization.

7)  NOT to alter the dog’s appearance in any way, shape or form (e.g. cropping, docking).

8)  To immediately notify Paws4Survival in the event of the dog’s death within the first year of adoption.

9)  To allow a Paws4Survival representative to make periodic visits to my home during the first six months following the signing of this agreement and to further visits if Paws4Survival deems it necessary.

10)  To immediately notify  Paws4Survival of any change of address or telephone number, or of any plans to move from my/our current residence during the first year from the signing of this agreement, and notify Paws4Survival of any change in circumstances which may prevent me/us from keeping the dog or comply with any provision of this agreement.

11)  I/we understand that the dog has been checked, and vaccinated by a veterinarian and that his/her state of health is accurately represented to the best knowledge of Paws4Survival. However, if I/we wish to, we can have the dog examined by my/our own veterinarian within ten days of adoption.  If upon such examination it is determined that the health of the dog is not satisfactory, I/we may return the dog within a week ,or a more reasonable amount of time, to allow  Paws4Survival to secure boarding or a foster home.  In any event, I/we understand and agree that if I/we choose to return the dog, the adoption fee is not refundable.

12) I/We understand that Paws4Survival will follow up on an annual basis to ensure the dog is on monthly heartworm prevention, flea tick prevention, up to date on vaccines and brought to the vet at least once a year for annual wellness visits. If it is determined this is not being done Paws4Survival has the right to take ownership of the dog back.

I/we understand that no representations are made by Paws4Survival as to the temperamental or mental disposition of the dog.  Accordingly, I/we hereby adopt the dog at my/our own risk, and indemnify and release Paws4Survival, its agents, volunteers, Officers and Directors, of any and all liability arising from damages to person(s) or property caused by the dog.
